Need a medical certificate for work?
Wellworks Pharmacy offers a convenient online medical certificate service for people who have experienced a recent minor or short-term illness and require a certificate for work.
Complete the online assessment below and our pharmacy team will review the information you provide.
Cost: $35
Please note that this is a general assessment for people with mild to moderate symptoms. If you are feeling very unwell then we recommend you see a doctor for a more thorough assessment.
Is this service suitable for me?
This service is suitable for people who:
Live and work in New Zealand
Have experienced a mild, short-term illness
Require a medical certificate for their workplace
Require a medical certificate for up to 3 days
Have an illness or condition that can appropriately be assessed within a pharmacist’s professional scope
This service is not suitable if:
You require a medical certificate for more than 3 days
You require an ACC medical certificate
You require certification for compassionate leave
Your illness or injury requires assessment or treatment beyond the pharmacist’s professional scope
If the pharmacist considers that you need a more thorough assessment, you will be referred to your doctor or another appropriate healthcare professional.
We recommend checking that your employer will accept a medical certificate issued by a pharmacist before using this service.
How does it work?
1. Complete the online assessment
Complete the short questionnaire below and provide some information about your recent illness.
2. We review your information
Our pharmacy team will review your form after it has been submitted.
3. We contact you
We will get in touch with you to confirm the information provided and make sure the service is appropriate for you.
If further assessment is required, we will let you know.
4. Pay the $35 service fee
Once everything has been confirmed, we will send you a text message with a link to pay the $35 medical certificate fee.
5. Receive your medical certificate
Once payment has been received and the pharmacist is satisfied that a medical certificate can appropriately be issued, your medical certificate will be sent to you electronically.
Important information
Completing the online questionnaire does not guarantee that a medical certificate will be issued.
Medical certificates are issued at the pharmacist's professional discretion based on the information provided and any follow-up discussion with you.
If the pharmacist considers that your condition requires a more thorough clinical assessment, we may recommend that you see your GP or another appropriate healthcare professional.
This service is not intended to replace medical assessment where you are significantly unwell, your symptoms are worsening or you have concerns about your health.
